In an alarming and rare medical case, a 58-year-old man from Haikou, unwittingly inhaled a cockroach while sleeping, causing days of discomfort. The man, who remains unnamed, sought medical help after three days of persistent coughing and a strong foul odour in his mouth. To his horror, doctors discovered and removed a cockroach from his trachea during a bronchoscopy.
How It Happened
The man recounted the strange incident to doctors at Hainan Hospital. Three days before his hospital visit, around 4 a.m., he felt something crawling into his nostril as he slept. Startled, he coughed briefly but thought little of it at the time. However, the following days brought persistent coughing, yellow phlegm, and an unpleasant taste and smell in his mouth—one he described as distinctly resembling cockroach odour.
Medical Investigation and Discovery
Dr. Lin Ling, a respiratory and critical care specialist at Hainan Hospital, took charge of the case. Initially, the man had visited an ear, nose, and throat (ENT) clinic, but no foreign object was detected in his upper respiratory tract. This led doctors to suspect that the object had moved deeper into his trachea or bronchial tubes.
A CT scan confirmed their suspicions, revealing a shadow in the lower right lobe of his lungs.

Given the patient’s discomfort, the medical team promptly conducted a bronchoscopy, a procedure that uses a thin, flexible tube to examine the airways and lungs.

During the procedure, Dr. Lin was shocked to discover a cockroach lodged in the man’s bronchus. The insect was wrapped in mucus, making it harder to detect initially. After carefully suctioning out the surrounding phlegm, the insect was fully exposed and removed.

“The procedure went smoothly, and we made sure to flush out all the surrounding secretions to ensure there was nothing left behind,” Dr. Lin said. After the operation, the man reported feeling significantly better, with his cough and phlegm production reduced, though he still experienced a slight lingering odour.
Prevention Tips
This unusual case highlights the importance of maintaining a clean-living environment, especially in tropical regions like Hainan, where cockroach infestations can be common. Dr. Lin emphasized that household hygiene and timely pest control can prevent such incidents.
Additionally, individuals with a weakened cough reflex, especially those who may be older or have neurological conditions, should consider seeking medical advice to reduce the risk of foreign objects entering their airways during sleep.
While inhaling a cockroach is a rare event, this case serves as a reminder of the unexpected challenges pests can pose to human health.
